Carepaths is an electronic health record where is maintain your assessments, notes and other documents. Carepaths will send text or email reminders to you the day before your appointment. You can pay your co-pay directly on Carepaths, send me emails and have video telehelath session.
When you become a client, I require that you sign up for Spruce Health for communication and I will provide you with log in information to access your health records on Carepaths.
All interactions with Jackie Boggs, LMFT, including scheduling of or attendance at appointments, content of your sessions, progress in counseling, and your records are confidential. You may request in writing to release specific information about your counseling to persons you designate.
EXCEPTIONS TO CONFIDENTIALITY:
• If there is evidence of clear and imminent danger of harm to self-and/or others, I am legally required to report this information to the authorities responsible for ensuring safety.
• Oregon state law requires that I must report information if I learn of, or strongly suspect, physical or sexual abuse or neglect of any person under 18 years of age I must report this information to county child protection services.
• A court order, issued by a judge, may require me to release information contained in recordsand/or require me to testify in a court hearing.
